Documentation NEEDED

  1. -Driver License
  2. -Social Security Card
  3. -State of Florida Licensure
  4. -CPR card
  5. -One year experience
  6. -HIV/AIDS certificate
  7. -OSHA certificate
  8. -Domestic Violence certificate
  9. -Auto Insurance
  10. -Level II Background Screening
  11. -Proof of Citizenship/Residency(Voter Registration, Resident Card, etc.)
  12. Proof of Liability Insurance



 

*Nurses must carry their own liability insurance and the policy has to be active.

Most nurses that come to us are insured through Nurses Services Organization (NSO.com). Insurance rates vary, but usually range from $50 – $100/year.
